Online appointment booking is handled through an approved secure scheduling service. Information entered in the scheduling flow is processed separately from the general contact form.
Appointment communications
Appointment confirmations and communication preferences are handled through the scheduling workflow and the practice. Contact Forward Care if you have questions about appointment communications or need to update a preference.
Patient forms
Registration, telehealth, records-release, privacy-notice, and terms forms are displayed from Forward Care’s existing secure form destinations. Information entered into those forms is submitted through the practice’s approved configuration; the website does not separately store the form responses.
Protect your medical information
Submit medical information only through the approved patient portal, appointment scheduler, or secure patient forms. Do not include private or sensitive medical information in ordinary email or the general contact form.

Forward Care is committed to protecting patient information and to handling protected health information in accordance with HIPAA and other applicable privacy and security requirements.
